By this agreement the service provider agrees to fulfill the duties involved with maintaining the OSS as prescribed by the system’s manufacturer and this agreement. The homeowner also agrees to fulfill the duties assigned to them as prescribed by this agreement.

This agreement will be initiated on 03/26/2025, and will be effective for 1 year.

The inspection procedure will be clearly outlined in the following pages. Any additional, unscheduled inspections, as well as the diagnosis and repair of any problems discovered during an inspection by the service provider or any other persons are not covered under the cost of this agreement. An additional fee will be charged, but only after being authorized by the homeowner. Work performed by an unaffiliated service provider is not covered under the terms of this agreement.
Duties of Service Provider:
The service provider will perform two inspections per contracted year. The inspection will include a service check, involving all recommended criteria. An example of a service log is included with this agreement.
All findings will be reported to the homeowner promptly. The inspection report will be either left with homeowner at the completion of the inspection, on the day of the inspection, sent by means of the postal service, or sent electronically through email. Whichever is preferred by the home owner.
The service provider will immediately report any problem discovered during the inspection to the home owner as well as a method for repair and the cost involved.
Should a problem be detected by the home owner and a service inspection requested, the service provider is required to respond to the request within 2 business days.
Duties of the Home Owner:
The home owner is responsible to notify the service provider of any problems discovered with the OSS in a prompt manner. If at any time during the contracted year, the OSS is in need of repair, the service provider will promptly provide an approved method of repair as well as the cost involved. However the home owner may decide to get another contractor to assess the problem and provide an additional quote. Retrieving an additional quote would be the home owner’s responsibility.
The home owner is responsible to keep the monitoring/ access covers free of obstructions at all times.
